Doc2Form
Convert any PDF or document into a Google Form with AI
About Doc2Form
Doc2Form converts documents into Google Forms. You upload a PDF (even a scanned one), a Word document or a photo of a paper form, or simply describe the form you need in plain English, and Gemini AI extracts the questions, detects the field types and section breaks, and creates a native Google Form directly in your Google Drive. It removes one of the most tedious office and classroom chores: retyping an existing questionnaire, intake form, survey or quiz into Google Forms question by question.
Doc2Form deliberately does one thing. Rather than exporting to a dozen form builders or learning platforms, it builds real Google Forms through the Google Forms API, so the result can be edited like any form you made by hand, shared by link or embed, and connected to Google Sheets for responses. It handles legacy scans, photos and messy PDFs with AI vision where plain text tools fail, detects more than 15 question types including rating scales, Likert and NPS, and reports an average conversion time of 34.2 seconds. Uploaded files are used to extract structure and then discarded rather than stored. The project is open source under the MIT license: you can self-host it free with Google Apps Script and your own Gemini API key, or use the hosted version at doc2form.dev, where the first form is free and further forms are bought as credit packs that never expire.
On the hosted version you sign in with Google at doc2form.dev, with no Workspace Marketplace add-on to install. You then drop in a PDF or Word document, including many scanned PDFs and photos, or type a plain-English description of the form you want. Optional instructions let you steer the result.
Gemini AI reads the document and identifies the questions, the answer options, the field types such as multiple choice, dropdowns, checkboxes, short answer and scales, and the section breaks.
The Google Forms API then creates a native form in your own Drive, usually in about a minute. You open it in Google Forms, edit anything you like, connect it to Sheets and share it.
If you prefer to run it yourself, you set up your own instance from the GitHub repository with Google Apps Script and a free Gemini API key. That version has no server, runs inside your Google account and creates unlimited forms; the site says setup takes about five minutes.

Doc2Form is made for teachers turning worksheets and tests into self-grading Google Forms quizzes, HR teams digitizing onboarding and feedback forms, and researchers converting survey instruments into Forms. Clinics, legal teams and agencies with stacks of paper or scanned forms can digitize them without sending sensitive files to long-term storage. Event planners and marketers can build registration and feedback forms from existing documents. Teams that only convert a handful of forms a year can pay per form, and technical users or schools who want unlimited conversions can self-host the open-source version.
